Counter Culture: What the Clinic Counter Sells, With the Lists Read

These are the brands you meet in a treatment room rather than a beauty aisle. Sold through a practice, or a clinic, or a website that wants to know who referred you. Sixteen of them here, from forty six dollars to two hundred and ninety five, and the whole shelf comes to a little over two thousand two hundred.

Before you spend that, here is what is in them. We read every ingredient list these brands publish, and all sixteen now publish one. Seven of them name fragrance or parfum outright. Five declare an allergen on the label: limonene, linalool or citronellol. An eighth, the PCA toner, avoids the word fragrance and then lists clove, cinnamon and ginger oils. All four ZO products are fragranced. So is the Murad night cream, and so is the neck cream you are meant to use every night on the thinnest skin you own.

None of that makes them bad, and we would not have built a shelf out of them if we thought it did. It does mean the clinic counter is not the low irritation tier people assume they are paying for. Eight have a full list with no fragrance in it at all: the iS Clinical cleanser at forty nine dollars, the Jan Marini serum, the BIBA de Sousa serum, which has fourteen ingredients in total, the SkinMedica and SkinCeuticals serums, the Metacine, and both Plated serums.

The one on this shelf we would hand to anybody. No fragrance, no parfum, nothing declared, and the actives are willow bark, sugarcane, centella and green tea rather than a surfactant system doing all the work. On a shelf where eight of the sixteen lists we could read carry fragrance, it is one of the eight that do not, and it is by far the cheapest of those eight.

The scrub that made ZO's name, and a fair introduction to what this whole shelf is like. The crystals are magnesium rather than a shell fragment, so they are uniform and rounded instead of jagged, and they sit in an oil base that keeps them moving over skin. It is also, like the rest of the ZO range, fragranced, with limonene and linalool declared on the label. That is worth knowing before you scrub with it.

PCA Skin Nutrient Toner

Toners USA Approx. $46

One of the shortest ingredient lists on this shelf, which is usually a good sign and here comes with a catch. Lactic acid, glutathione and a pumpkin ferment do the work. Then clove leaf oil, cinnamon cassia leaf oil and ginger root oil, with denatured alcohol. There is no parfum on the label, but three warming essential oils is a fragrance by any sensible reading, and it explains why it smells like it does.

Salicylic acid pads with alcohol second in the list, which is the formula that makes them work quickly and the formula that makes them a poor idea for anyone reactive. ZO replaced the microplastic pad material with a plant-based one, which is a genuine improvement nobody was forced to make. Three fragrance allergens are declared here: limonene, linalool and citronellol. Of everything on this shelf, this is the one to be most careful with.

What we can tell you is the formula is silicone-led, which is why it blurs, and that parfum is in the list.

The most famous serum in this whole category and the one people mean when they say they use a growth factor serum. The dual chamber pump is the real design idea: the two textures stay separate until the moment you use them. The list reads as advertised, human fibroblast conditioned media in first position after water and glycols, with a dimer tripeptide, whey protein and a chlorella extract behind it, and no parfum anywhere, which confirms the brand's fragrance free claim rather than just repeating it. At two hundred and ninety five dollars it is the most expensive thing here.

It is also one of eight things on this shelf whose full list carries no fragrance and no parfum, among them the iS Clinical cleanser and the BIBA serum. At a hundred and ninety eight dollars that combination is rarer than it should be.

A Beverly Hills plastic surgeon's brand, sold mostly as a three-part set that runs to eight hundred and seventy five dollars. The Plasma is the first step and the one you can buy alone, which is why it is the one here. It belongs on this shelf for what the list actually contains: five of the lab-named peptides the category calls growth factors, a tobacco-plant-derived hexapeptide, copper tripeptide-1 and hyaluronic acid, in a fluid the brand says is modelled on platelet-rich plasma. That is the brand's framing, and it is the clearest example on the page of skincare using a clinic's vocabulary. (plated) builds its formulas around what it calls Renewosome technology, and the eye serum is the cheapest way to try it. We name the technology the way the brand does and stop there, because this is a young category and we are not going to tell you what it does. The list itself is published and unremarkable in the good way: human platelet extract, hyaluronic acid and panthenol, with two palmitoyl peptides, coffee seed extract and carnosine. An eye product is the sensible place to test an expensive idea, since the area is small and a bottle lasts.

Same position as the eye serum: we name the technology the brand names and we do not tell you what it does. The published list is shorter than the eye serum's, twelve lines of human platelet extract, hyaluronic acid, panthenol and glycols, with none of the peptides the eye version carries. If you want to try (plated) and you are not sure, start with the eye one.

Two forms of vitamin A rather than one: retinol and retinyl propionate are both in the list, alongside niacinamide and sodium hyaluronate. That is a sensible way to build a night cream, because the cushion and the retinoid arrive together rather than as two steps you have to balance. It is fragranced, and citronellol is declared, which for a nightly retinoid product is worth weighing.

What we would want to know before putting it on our neck every night is that it carries fragrance and parfum, declares limonene and linalool, and has bergamot peel oil in it. Neck skin is thinner than face skin and it is the part people most often say reacts.

The heaviest moisturizer on the shelf and the one you reach for when a retinoid has gone too far. It is an overnight cream in the old sense: you put it on last and you feel it there. Obagi does not publish the list on its own page, which is why this entry named nothing for months. Dermstore does publish it, and read in full it is a butter-and-oil cream, shea and mango seed butter over avocado oil, with sodium hyaluronate, allantoin, bisabolol and two peptides further down. It also contains fragrance, last on the list.

The clearest case on this shelf of a product designed around an appointment rather than a routine. It is SkinCeuticals' aesthetic recovery cream, formulated for the weeks after a procedure, and increasingly it is the tube a provider hands you on the way out of the room. Judge it on that job. As a daily moisturizer at a hundred and ninety five dollars it makes far less sense.
